Lunch comes with the two long days out, and costs about $10 to 15 a head on the Centro Histórico walk. Either way you eat somewhere cheap and local and you order it yourself, in Spanish. That is the lesson, not a formality.

It helps to have a few classes behind you, because the whole point is using what you have. Complete beginners are welcome but the day runs slower and your teacher translates more. If you are unsure, take a free online class first and ask.
The route changes. Museums, markets and cafés work just as well as pyramids for a day of talking, and your teacher decides in the morning. You do not lose the excursion.
Yes, up to 4 people in total. The price shown is for one person; each extra person is $50 on Centro Histórico, $50 on Xochimilco and Coyoacán, $80 on Teotihuacán and la Villa. That covers their entries, transport and water, and Teotihuacán costs more because every extra body pays its own way into the site. They do not need to speak Spanish: the beginner listens, you translate, and translating out loud is its own lesson.
On Xochimilco and Coyoacán and on Teotihuacán and la Villa, yes: lunch is included, at a cheap local place rather than anywhere that speaks English. On the Centro Histórico walk it is not, and you pay around $10 to 15 a head for your own. Either way you order it yourself, in Spanish, which is the point. Water and snacks are on us all day. Tell us about any allergies beforehand and we will pick the place around them.
A week is comfortable, especially for Teotihuacán. Same-week requests sometimes work, so ask anyway.
